Out-of-Network:
Some PPO plans offer out-of-network coverage. I will provide you with a statement you can submit to your insurance for reimbursement.
Payments can be made via Venmo, Paypal, check or cash.
*I will verify I am in network with your insurance plan, but due to the complexity of individual insurance plans it is up to you to inform yourself about the details of your plan’s coverage. Some plans include deductibles that must be met before coverage starts.
